State Waiver Programs
The NHTD Waiver Program is a home and community-based program that helps New York’s Medicaid-Eligible seniors and people with physical as well as cognitive disabilities receive the comprehensive services they need while they live in a community-based setting.
The NHTD/TBI Waiver Program provides Service Coordination which is an individually designed service that provides primary assistance to the waiver applicant/participants in gaining access to the needed waiver and Medicaid State Plan services.
We also assist with other local state and federally funded educational, vocational, social, and medical services.
our Service Coordinator assists the applicant in becoming a waiver participant and coordinates and monitors the provision of all services in the service plan once the individual is determined eligible..
We have qualified Service Coordinators who are bilingual and can deliver service coordination in various languages such as English, Spanish, and Russian, taking into consideration the cultural diversity of New York State.
